PARTICIPANT/VISITOR LIABILITY RELEASE AND WAIVER 

This Participant/Visitor Liability Release and Waiver form (‘Release’) is entered into as of the date set forth below, by and between the Humane Society for Greater Lafayette (HS4GL) and the person(s) named below (‘Undersigned’). 

WHEREAS, HS4GL is a non-profit organization dedicated to the rescue of animals in need, HS4GL has any number of rescue animals in its possession on the premises at 3150 Sagamore Parkway S. Lafayette, IN at any given time. 

The Undersigned acknowledges that the premises of HS4GL, and the animals, guests, items and activities thereon, can pose dangers to people and animals. Such dangers include, by way of example and not limitation, animals that can nip, bite, trip, knock down, fight, indoor and outdoor surfaces may be slippery and/or steep, equipment that can trip, activities involving animals that can result in injury from bites or other causes. Such activities can be dangerous, unpredictable, and involve health risks. The Undersigned covenants that he/she is healthy enough to participate in such physical activities. Accordingly, and in consideration of being able to participate with and/or visit HS4GL, the Undersigned, on behalf of himself/herself and his/her minor child/children, releases HS4GL and the Owner of the Premises, and their employees, affiliates and assigns, from and assumes the risk of injuries, losses, damages, costs and expenses of every possible cause and description unless inflicted intentionally or recklessly by the agents and/or employees of HS4GL or the Owner of the Premises. The Undersigned also agrees to defend and indemnify HS4GL and the Owner of the Premises, and their employees, for any claims made against HS4GL by the Undersigned and/or his/her minor child/children.
